Modafil MD 100 mg is a mouth-dissolving tablet formulation containing Modafinil, manufactured by Intas Pharmaceuticals Pvt. Ltd. Modafinil is an eugeroic used primarily to manage excessive daytime sleepiness associated with sleep disorders. Mouth dissolving formulation is designed to disperse rapidly in the mouth, offering an alternative for patients who have difficulty swallowing conventional tablets.

MECHANISM OF ACTION

  • Dopamine Reuptake Inhibition: Modafinil weakly inhibits the dopamine transporter (DAT), increasing the levels of dopamine in the certain regions of the brain.
  • Histaminergic and orexin/hypocretin system activation: Modafinil increases the activity in the hypothalamic histamine and orexin neurons which plays an important role in maintaining wakefulness.

  • Hormonal contraceptives: Modafinil induces CYP3A4 and may reduce the effectiveness of hormonal contraceptives (pills, patches, implants). Alternative or additional contraception is recommended during and for some time after stopping the drug.
  • Anticoagulants: Increased monitoring may be needed as modafinil can affect clearance.
  • CYP3A4 substrates: May reduce the plasma concentrations of CYP3A4 substrates (e.g., ciclosporin, some steroids).
  • CYP2C19 substrates: Modafinil inhibits CYP2C19 and may increase the plasma concentrations of drugs such as phenytoin, diazepam, propranolol and some antidepressants (e.g., clomipramine).
  • CNS-active drugs: Use caution if taking other stimulants, MAO inhibitors, or other drugs affecting dopamine pathways.
  • Alcohol: Alcohol may affect the wakefulness-promoting effect of modafinil and impair judgement.

Patients should inform their physician regarding the medications, herbal supplements, etc that they are taking.

PRECAUTIONS & WARNINGS

  • Modafinil should only be used under medical supervision with periodic review of continued need.
  • Use caution in patients with a history of cardiovascular issues such as angina, arrhythmia left ventricular hypertrophy, etc.
  • Rare but serious hypersensitivity reactions including Stevens-Johnson disease, DRESS, etc have been reported.
  • Modafinil is not recommended in pregnant women as it is associated with increased risk of congenital malformations.
  • Dose adjustment or caution is advised in renal or hepatic impairment.
  • Although modafinil promotes wakefulness, underlying sleep disorders themselves can impair alertness; patients should assess their individual response before driving or operating machinery.
